help please advice required .re continuing healthcare

hello
my father has been in hospital since 21st oct 2011. he is 88 years old and has been diagnosed in hospital as having vascular dementia. this has caused him to be immobile and he is on at least 0.5-1mg of lorazepam as he is voilent and aggressive. plus they have a nurse specialing him.
the hospital discharge team want him to go into an emi home .the doctor say he will die with it but cannot give a timetable. he has low sodium and his blood pressure is up and down like a yo yo. they have stopped treating these two conditions as they say the vascular dementia is causing them and the treatments were not working. the consultant also said to my brother at a previous meeting that if my father gets any illness that requires antibiotics then they will not give him them as it would be cruel to prolong. i just need advice as to wether he would qualify for chc. our choice would be for him to remain in hospital . thanks
